A lifetime of dieting, exercise and healthy lifestyle is required for long-term success after bariatric surgery. A physician-supervised program consists of nutrition consoling, increased physical activity and behavioral modification.

There must be medical record documentation of program participation by the attending physician of the organized program or the patient's primary care physician. Documentation should include the patient's progress or lack of progress. Most insurers also require that the problem of morbid obesity is at least two years old. At least one attempt of a medically-supervised diet must be documented for at least six consecutive months in a one-year period prior to the request (predetermination) or date of surgery. Six months does not equate to six physician visits.
